Complex Care Management Program Staff

The Complex Care Management Program (CCMP) was formed in 2005 to coordinate care for UMHS patients with the highest level of need, including physical illnesses, mental illnesses, homelessness, and lack of adequate health insurance.

Brent Williams, MD is the Medical Director of the Complex Care Management Program (CCMP), Dr. Williams has developed clinical programs to improve care and decrease inappropriate health care expenses among high-utilizing patients with combined medical, mental health, substance abuse disorders, and/or poverty and homelessness. Complex care management programs have been developed in collaboration with the Office of Clinical Affairs, Michigan Medical Home Work Group, primary care practices, and the Office of Population Health headed by Dr. Maria Han.

Dr. Williams is the primary liaison for Michigan Medicine with community mental health (CMH) services, and co-chairs a task force developing joint clinical programs between Michigan Medicine and CMH. He also serves on the Washtenaw Health Initiative (WHI) Steering Committee and on multiple WHI work groups related to safety net clinics and mental health services.

In medical education, Dr. Williams serves as Director of the UM Medical School Global Health and Disparities Path of Excellence, a co-curricular pathway providing medical students the opportunity to participate in summer research activities across the globe and develop leadership skills through in a series of community-based learning experiences, seminars, and structured learning experiences.

An active primary care physician, Dr. Williams has published articles related to medical education and faculty development, and recently co-edited a book on the care of hospitalized older patients.

Donna Fox, RN is the Manager of the Complex Care Management Program. Donna has an Associate of Applied Science degree in Nursing. She has been with Michigan Medicine since 2006. As the CCMP Manager, Donna oversees the daily work within the program and ensures that all aspects run smoothly. When not at work, Donna enjoys spending time with her children and grandchildren and rooting for the Ohio State Buckeyes.

Heather Rye, LMSW is the Program Supervisor and a Care Manager for the Complex Care Management Program. In this role she provides direct supervision and oversight to Complex Care Managers and Complex Care Manager Assistants, as well as continuing to manage a small caseload of patients. Heather is passionate about advocating for the underserved. She has been with the Complex Care Management Program since 2009.  Heather has a Masters in Social Work from Wayne State University and is a Certified Case Manager. When not at work, she enjoys spending her free time with her family.

Kelly Reid is the Administrative Assistant Intermediate for the Complex Care Management Program. She is also the department’s Diversity, Equity and Inclusion Lead.  Kelly began working at Complex Care, as a Care Manager Assistant, in July of 2010.  Prior to coming to Michigan Medicine, Kelly had several years of administrative experience working in a large outpatient Psychiatric office and she is trained as a Medical Assistant. Kelly’s favorite things about working at Complex Care are the unique, non-biased, longitudinal care that CCMP provides to their patients and working closely with her amazing team members. In her spare time, Kelly enjoys being with her family and friends, being artistic, practicing yoga and playing golf.

Complex Care Managers



Michelle Adgate, LMSW has been a Complex Care Manager at Complex Care Management Program since 2015. She received her master’s degree in Social Work from the University of Michigan in 2008 and has worked with underserved populations doing intensive case management since. These populations include those who are homeless and struggle with mental illness and addiction. Michelle’s favorite things about working for Complex Care include the opportunity to work with underserved patients, having the ability to be challenged and learn something new every day, and being surrounded by her amazing, supportive colleagues. Outside of work, Michelle enjoys being outdoors, spending time with family and cooking.

Ashley Beasley, BSW Ashley Beasley is a Care Manager with the Complex Care Management Program. She joined CCMP in 2014. Ashley earned her Bachelors in Social Work from Eastern Michigan University and is currently working on her Masters in Social Work at Eastern Michigan University, with a concentration in Mental Illness and Chemical Dependency. Ashley really enjoys that her role within Michigan Medicine allows her to provide patient-centered care for individuals with the highest level of need, in order to help improve their overall quality of life. In her spare time, Ashley enjoys spending time with family and traveling.

Thaddeaus Jabzanka, LMSW is a Care Manager in the Complex Care Management Program. He joined the Complex Care Management Team at the beginning of 2016. Thaddeus earned his Bachelor's degree in Ethics, Social and Political Philosophy from The University of Massachusetts Boston and received his Master's degree in Social Work from The University of Michigan in 2011. In this role as a Care Manager, he works with Michigan Medicine patients who need significant medical treatment and have multiple barriers to care and living independently, working in partnership with the patient to assist them in reducing those barriers and to better navigate the healthcare system as well as community resources available to them. When not at work, Thaddeus enjoys listening to music, biking, hiking and watching U-M athletics.

  Thaddeaus Jabzanka

Measie James, MSW, MSA is a Care Manager with the Complex Care Management Program. She joined the Complex Care Management Team in 2017. As a Care Manager, Measie coordinates care and helps meet the medical and social needs for some of Michigan Medicine’s most complex patients. Measie completed her undergraduate degree at Tennessee State University, where she earned a degree in Speech Pathology and Audiology. She also attended Central Michigan University where she obtained a Master of Science degree in Health Care Administration and later completed her Master of Social Work degree at the University of Michigan.  In her spare time, she enjoys spectating sports, cycling, volunteering and spending time with family and friends.

  Measie James

Doug Montgomery, MS is a Care Manager for the Complex Care Management Program. He earned his PA from Michigan State University and his MS from the University of Detroit Mercy. Doug has been with the Complex Care Management Program since 2013. As a Complex Care Manager, Doug coordinates care and services for the health system’s most complex, at risk and vulnerable patients. When not at work, Doug enjoys folk music, bonsai, and golf.

Joseph Petricca, LMSW is a Complex Care Manager who started with the Complex Care Management Program in 2011. He completed his Bachelors of Science with a major in Psychology at Eastern Michigan University and his MSW at Loyola University-Chicago. He has experience working with individuals with dementia, running individual and group therapy, providing treatment in residential Mental Health programs and is a Certified Case Manager. He works to empower and advocate for patients, provide support to the health system, ensure quality care and works to build community supports. His favorite part of working at CCMP is the camaraderie with his team, and connecting patients to resources to enhance their self-care. Joe enjoys playing music, spending time with his family and reading in his spare time.

  Joseph Petricca

Erin Stanley, MS is a Complex Care Manager who started with CCMP in 2013. Erin has a Masters of Arts from Eastern Michigan University, a Bachelor of Science from Loyola University Chicago, and is a Certified Case Manager. Prior to coming to CCMP, Erin worked for the Shelter Association of Washtenaw County doing outreach and medical case management through their health clinic. Her favorite part about CCMP is working with the team on implementing a harm reduction approach to healthcare as well as building good relationships with her patients. Outside of work, Erin enjoys traveling, outdoor adventures, and all things Detroit.

Tara Truax, LBSW came to the Complex Care Management team from the University of Michigan Medical School Standardized Patient Program in 2016. Ms. Truax previously worked with the Michigan Department of Health and Human Services as a Housing Programs specialist and at Washtenaw County Community Mental Health as a Client Services Manager. Prior to this, she worked in outreach and permanent supportive housing programs with nonprofit agencies in Ann Arbor and Ypsilanti, MI. In her role as a Complex Care Manager, Tara provides assistance in coordinating medical care, linking patients to needed resources, and helping to support the patient’s care team to ensure that quality care is provided. Tara attended Eastern Michigan University and is a Licensed Social Worker. In her free time, Tara enjoys playing with her cat Phoebe, being outdoors, travel, and exercise.

Skilled Nursing Facility Care Manager

Elizabeth (Liz) Aslin, LMSW, MPA is a Skilled Nursing Facility Care Manager for Michigan Medicine. Liz has had experience as a hospice social worker, psychiatric emergency social worker and most recently as the social work supervisor at a residential facility for adults and children with developmental disabilities. She joined Michigan Medicine and the Complex Care Management Program as a SNF Care Manger in 2016, for a new program that works to coordinate the transitions of patients from Michigan Medicine to Sub-Acute Rehab, then to home.  In this role, Liz works to support those who are navigating the complicated health system and to assist them in transitioning back to the community.  She attended Seton Hall University for her Bachelor’s in Social Work and a Master’s in Public Administration.  She then attended Rutgers University for her Master’s in Social Work.  In her spare time, Liz enjoys spending time with her children, gardening and running.

Care Manager Assistants

Samantha Bachoura is a Care Manager Assistant with the Complex Care Management Program.  She studied Sociology at Eastern Michigan University and has been with Michigan Medicine since 2014. In 2017, Samantha joined the Complex Care Management team.  She plans on using her experience at the Emergency Department, Wound Care Clinic and assisting with the Genes for Good research project to help patients thrive in the health system. As a Care Manager Assistant, Samantha strives to provide compassionate care for her patients as individuals with different visions of quality health care. In her free time she enjoys meditation, the arts, exploring new cities, and attempting to crochet near her cat.

Madeline Mehall is a Care Manager Assistant for the Complex Care Management Program. She has been a part of the team since 2017. Madeline earned her Bachelor’s in Health Systems Management at Loyola University Chicago. She is continuing her education at Wayne State University, working her way towards obtaining a Master’s in Public Health. Madeline’s favorite thing about working at Complex Care is working with an incredibly passionate team that advocates for all patients. In her spare time, Madeline enjoys being with friends and family, traveling, and running.

Debra Ridley is a Care Manager Assistant with the Complex Care Management Program.  She began working at Michigan Medicine in 1989 and has held several different positions throughout her career here. In 2007, Debra joined the Complex Care Management Program. As a Care Manager Assistant, Debra provides excellent care and support to her patients and team members. The Complex Care Program gives Debra the opportunity to do what she is most passionate about: reaching out to those in need.  Debra is a Michigan State Spartan fan who enjoys spending time with friends, watching sports and playing golf.
