To Make an Appointment
To schedule a patient appointment, please call M-LINE at 800-962-3555. You can also complete a Consult Request Form and fax it directly to the clinic to request a patient appointment.
Pediatric Services Online Consult Request Form
Physicians can also complete an online Consult Request Form to request an appointment for the Divisions of Pediatric Cardiology, Endocrinology, Gastroenterology, Nephrology and Pulmonology. This online form was especially designed to allow you to request appointments for Pediatric services at your convenience.
